Additional Membership Discounts
- Additional Exams: Limited/TAL exams (D0140) with diagnostic X-ray (D0220), $100 per exam
- 20% off any additional treatment needed in-office, including restorations, crowns and bridges, and dentures
- Nitrous oxide is not included in membership pricing and must be purchased separately at full cost, $127 (D9230)

Annual fee due at enrollment, payable by cash, check, or credit card. Financing of membership fees is not available.

- Benefits may only be used with Andresen Prosthodontics' assigned dentists.
- Andresen Prosthodontics is not a licensed insurer, health maintenance organization, or underwriter of health services.
- This plan cannot be combined with dental insurance, other offers, discounts, other discount plans, or advertisements.
- Membership benefits run for 12 consecutive months from the date of enrollment.
- Membership fees are non-refundable and cannot be transferred to other family members.
- Andresen Prosthodontics reserves the right to modify, change, or discontinue the membership program, its terms, services, and fees at any time and without prior notice.
